3 Spray on insulation is not put inside cement walls.Don't you think that would kinda hinder the integrity of the cement?

4 There is a facade product system called 'STO' that includes a Styrofoam like insulation material used here on some larger buildings. I'm pretty sure that there are other similar systems.

It is not structural in itself, just a covering up of the structural support elements.

10 On a pedestrian bridge at the Excaliber Hotel and Casion in Las Vegas I saw a spot where the veneer was peeling away and revealing that the material under it was styrofoam. The veneer was supposed to look like stone I think, like a medieval castle. Seeing the styrofoam under it wasn't shocking because, after all, it was Vegas. Dunno what was under the styrofoam.
