Almost as soon as the DC-10 left the runway, it rolled over and crashed, exploding in flames. Before the fires were out, investigators found the plane’s left engine on the runway.

Maintenance procedures from McDonnell Douglas call for the engine to be dismounted before the support pylon is removed. Several airlines were detaching both at the same time to speed up servicing. Maintenance records showed that almost a month before the crash, the left engine had been removed and replaced. Perhaps the most unforeseeable factor was that the remounting was done over two shifts. The forklift holding the engine and pylon bled off during the shift change, dropping the engine slightly – while still attached at the rear pylon mounting. This caused the overstressing of the pylon. It took several more trips for the overstress to degrade to the failure.

The NTSB determined that the probable cause was the asymmetrical stall and the loss of stall warning and slat disagreement indication systems resulting from maintenance-induced damage to the left engine and pylon assembly. An unapproved maintenance procedure – a short cut - caused damage, resulting in a series of very unusual - and unplanned - failures. Good records were essential to finding the cause and preventing further tragedies.
