5 Surely not always really loaded, or people wouldn’t need those speed loader thingies or those replacement bullet holders (magazines?) that look so cool in cop shows when the hero drops the empty one out and slams the replacement into place before resuming banging away at the bad guy. But certainly guns should be assumed to be loaded, just to be safe. ;-)
