1 It was due to a mid-air with another KC-135R. The other aircraft landed safely with a damaged tail.

2 I understand that a typical KC-135 crew consists of three members: a pilot, a co-pilot, and a boom operator - (2) Officers and (1) Enlisted.
Yet this aircraft was crewed by, essentially, (2) complete crews.

A discussion of the accident by someone who seems to know about these things

He suggests the extra crew was due to an extended mission where they needed spare guys. The collision happened when they were getting into a tanking pattern, not during refueling. He also addresses parachutes.
