#4  I remember a line in an old noir crime novel, some character saying "That kid's never held a weed in his life." Which is to say, affecting smoking behavior unconvincingly.

So, yeah, it was a thing, going way back.

#3  Not necessarily, jpal. In those days holding a cigarette was considered a necessary fashion accessory — and at parties the addition of a glass of alcohol. My mother relates that she would start the evening holding an unlit cigarette and a glass of white wine — both still untouched when she fetched her wrap at the end of the night.
